오늘은 백내장 수술할 때 술자들이 마주하기 싫어하는 부분에 대한 이야기입니다.
렌즈라는 부분이 노화가 생겨 발생하는 것이 백내장이고
백내장은 모양이 이렇습니다.

모양만 봐도 제거해줘야겠다는 생각이 드네요
일단 백내장은 수술해줘야 하는 병입니다.
하지만, 백내장도 경우에 따라 하드 케이스가 있습니다.
바로 이런 경우죠..

보기만 해도 싫어지네요.
이런 경우 어떻게 하냐??
잘 수술해드립니다.
저는 이렇게 잡기도 하고

이렇게 꿰매주기도 하고

이렇게 걸어주기도 합니다.

경우에 따라 이런 것도 넣습니다.

CTR이라도 부르는데 저희 병원은 ringjet이라고
좋은 것을 쓰지요^^

오늘 관심 있게 본 발표 내용은

올해 발표된 내용으로 백내장의 위치가 불안정할 때 수술 전후 고위수차를 검사한 내용입니다.

이 기계를 이용해서 수술 전후를 비교해 보았습니다.

– The aberrometer provides an aberration profile of the entire eye, including the root mean square (RMS) value for total aberrations, HOAs, and LOAs. The corneal topography map provided the corneal aberration profile. The machine’s software calculates the corneal aberrations from the total eye aberrations to provide a separate display of
the internal aberrations. Although internal aberrations can arise from the corneal stroma right to the retina, the lens is usually a major contributor.9 The MTF at 10 cycles per degree (cpd), displayed by the ray-tracing aberrometer , was recorded preoperatively and 6 months postoperatively.

수술 방법은 이랬네요
1. Cionni modified capsular tension ring (CTR) and/or an Ahmed capsular tension segment (CTS) were used to fixate the capsular bag to the sclera, depending on the extent of subluxation.
2. A doublearmed 9-0 polypropylene suture (Prolene) or a 7-0 polytetrafluoroethylene
suture (Gore-Tex.) was used to fixate the modified CTR or CTS to the scleral wall.
3. A 1-piece hydrophobic acrylic IOL (Acrysof SA60AT, Alcon Laboratories, Inc.) was implanted in the bag in eyes in which the capsular bag could be preserved. Sutured scleral fixation of a 1-piece poly(methyl methacrylate) (PMMA) IOL or sutureless intrascleral glued fixation of a 3-piece hydrophobic acrylic IOL (Acrysof MA60AC, Alcon Laboratories, Inc.) was performed in eyes in which the capsular bag could not be preserved.
