Cycloplegic Refraction in Hyperopic Children: Effectiveness of a 0.5% Tropicamide and 0.5% Phenylephrine Addition to 1% Cyclopentolate Regimen

작년에 나왔던 한국논문입니다.

김안과에서 했던 연구인데

아이들 시력 검사 시에 눈에 약을 넣고 하는데요

기본적으로  어떤 약을 쓰는지 궁금하시죠..

궁금하면 500원…ㅎㅎ

알려드릴께요

이런약을 써요 어렵죠…

일단 아트로핀인 교과서적인 약이지만 20방울 이상 사용시 심정지가능합니다.

즉 죽을수도 있는 부작용이 있습니다.

그래서 잘 안씁니다.

저희병원도 아이들에게선 사용하지 않습니다.

호마핀 이약도 우리나라에서 구할수가 없는 약입니다.

왜 약가가 너무싸서 제약사들이 안만들어줍니다. 수입도 안되고 있고요.. 최근 한림제약이라는 회사에서

비보험으로 만들어 주고는 있지만 이 약 역시 작용시간이 너무길어 3일정도는 약때문에 안보이게 됩니다.

이러면 남은 약은 두개죠

cyclopentolate and tropicamide!!

cyclopentolate 는 시중에서 오큐시클로 1% 와 사이클로질 1%로 나와있습니다.

standard로 사용할수 있지만 이역시

부작용으로 심장이 빨리 뛴다거나 정신줄을 놓는 부작용 작용시간이 24시간정도 되죠..

최근 모안과에서 사용후 아이가 응급실에 실려간적이 있죠..

그럼 남은 약은 tropicamide하나 남았네요..

우리나라에선 산동제로 주로 쓰이는 mydrin-p에 0.5%tropicamide and 0.5%phenyephrine이 섞여있습니다.

즉 산동이 주요 효과이고 조절마비는 부수적인효과입니다.

하지만 전연령에서 안전하게 사용할수 있죠..

이연구에서는

두가지 약물군은 비교한 것으로

(Regimen 1 was a Tropherine-added regimen comprising the administration of one drop of 1% cyclopentolate followed by two to three drops of Tropherine added at 15-minute intervals.

Regimen 2 was a cyclopentolate-only regimen comprising the administration of three to four drops of 1% cyclopentolate at 15-minute intervals.)

내사시군도 비교해보았고

다크홍채라 불리는 우리나라 환아를 연구했습니다.(다크홍채는 약이랑 반응해  산동제의 효과가 떨어집니다. 즉, 외국아이들은 0.5% 로 충분하다면 다크홍채는 1%가 필요하다는 통념입니다.)

The mean difference (±standard deviation) in the spherical equivalent (SE) between cycloplegic and noncycloplegic refraction was significantly
larger in regimen 2 than in regimen 1, with values of +1.70 ± 1.03 diopters (D) and +1.25 ± 0.89 D, respectively (p = 0.001). The SE change after cycloplegia was significantly different between the two regimens only in patients aged 5 years or younger (p = 0.001), particularly in those with high hyperopia with an SE ≥5 D (p = 0.005) or fully accommodative esotropia (p = 0.009). There was no significant difference between the two regimens in patients older than 5 years, regardless of the presence of high hyperopia or fully accommodative esotropia.
