대전 민들레 안과와 알아보는 녹내장 이야기입니다.
최소 침습 녹내장 수술은 일반적으로 조직의 손상을 최소화하는 장치의 이식을 포함하는 수술 방법입니다. 합병증의 발생률이 낮고 수술 후 회복이 빠릅니다. 섬유주 절제술과 녹내장 배수 장치 (GABs)를 포함하는 전통적인 녹내장 수술과 비교됩니다.
1. iStent inject (Model G2)
2018년 FDA 통과를 받은 대표적 전방 삽입장치로
모양은 다음과 같이 생겼습니다.
결과도 좋게 나왔네요.. 그랬으니깐 FDA에서 통과를 시켜줬겠죠..
2세대 모델부터는 제조사 권고로 두 개씩 삽입하라고 하였으나, 1개만 넣어도 좋다는 보고입니다.
The iStent inject, as a stand-alone procedure, has demonstrated equivalence to treatment with two glaucoma medications, specifically latanoprost and timolol. Compared to baseline IOP, 94.7% in the iStent inject group (n = 94) maintained an unmedicated IOP reduction of at least 20% at 12 months compared to 91.8% in the medical therapy group (n = 98)

미국 IDE pivot study 결과로 백내장 수술 시 2개의 istent 삽입으로 2년 후 좋은 안압하강을 유지했다는 결과와 1세대한개를 넣어도 좋았다는 결과입니다.
At 24 months, 75.3% patients in the iStent inject cohort achieved at least 20% reduction in unmedicated IOP compared to 61.9% (P = 0.004) in the cataract surgery cohort, and the mean unmedicated IOP reduction was significantly greater (6.9 mmHg iStent inject vs. 5.4 mmHg for cataract surgery)

2. Hydrus Microstent
nitinol합급으로 만들어진 초승달 모양의 섬유주 바이패스 장치로 2018년 FDA 승인을 받았습니다. 8mm 길이로 삽입 방법은 동영상으로 보겠습니다.
HYDRUS II study, 100 patients with OAG were randomized to cataract surgery plus Hydrus Microstent implantation or cataract surgery alone. At 24 months, the Hydrus cohort demonstrated a greater proportion of patients with an IOP reduction at least 20% without ocular hypotensive medications (80 vs. 46%, P = 0.0008)

결과도 괜찮았습니다. 다른 스터디인 호리존 연구 결과에서도 백내장 수술만 받은 군보다 유의한 안압하강이 있었다는 보고입니다.
. At 24 months, 77.3% of patients in the Hydrus cohort demonstrated an unmedicated IOP reduction at least 20% compared to 57.8% for cataract surgery alone (P < 0.001).

3. MINIject
iSTAR 사에서 개발한 튜브 형식의 삽입물로 맹락막상강으로 방수 유출해서 안압을 낮추는 원리입니다.
간단한 원리 동영상 설명입니다.
사람을 대상으로 한 대단위의 연구는 아직 없지만, 2015년 25명을 대상으로 6개월 후 안압하강 효과가 좋았다는 보고도 있습니다.

4. iStent SUPRA
At 12 months (n ¼ 42), the postoperative mean IOP was 13.2 mmHg and 98% of these patients demonstrated more than 20% IOP reduction with reduction of one glaucoma medication.

5. InnFocus Microshunt
일본 산텐사에서 개발안 튜브 형태로 기존의 튜브 삽입술과 유사하며 결과는 좋다고 보고하였으며 최근 고식적 섬 우주 절제술과 비교하는 연구도 진행되고 있습니다.
Patients demonstrated a mean reduction of 10.7 2.8 mmHg (55%, year-1), 11.9 3.7 mmHg (50%, year-2), and 10.7 3.5 mmHg (55%, year-3).

물론 과거의 수술법이 나쁘다는 것은 아닙니다.
다만 최소절개수술이 대세인 현대 의학에서 안과도 이런 연구들이 활발히 되고 있습니다.
